
Both outdoor and treadmill walking confer similar core cardiovascular and metabolic training effects, but outdoor walking has consistently shown greater psychological benefits and higher enjoyment, while treadmill walking shows subtle physiological/biomechanical differences related to the fixed belt environment.[1][2][3]
Physiological and biomechanical differences
A systematic review/meta-analysis of 55 studies (1,005 participants) comparing treadmill vs. overground walking at matched speeds found that treadmill walking produces higher relative oxygen consumption and cadence, but shorter stride and step length, along with differing kinetic patterns — likely reflecting compensatory adjustments for reduced stability, discomfort, and unfamiliarity with the moving belt.[1] Despite these mechanical differences, a review of acute exercise bouts found the physiological benefits (cardiovascular, metabolic, thermoregulatory responses) were largely equivocal between outdoor and indoor exercise when matched for intensity.[3]
Psychological and well-being benefits favor outdoors
Multiple systematic reviews and meta-analyses show a consistent trend favoring outdoor/nature-based exercise for mental health outcomes:
- A systematic review of longitudinal trials found that of 25 statistically significant outcome comparisons between outdoor and indoor exercise, all favored outdoor exercise, though evidence quality was limited by small samples and heterogeneity.[2]
- A meta-analysis found nature-based exercise associated with greater positive affect, lower perceived exhaustion, and higher vigor, while indoor exercise was associated with higher perceived stress.[4]
- Earlier work found outdoor exercise linked to greater revitalization, positive engagement, and reductions in tension, confusion, anger, and depression, alongside increased energy — though feelings of calmness were sometimes lower outdoors.[5]
- A more recent network meta-analysis of RCTs found green exercise had a large, significant effect on mental health (depression, anxiety, distress) compared to control, and was significantly superior to indoor exercise (SMD −0.75), whereas indoor exercise alone showed only a small, non-significant benefit.[6]
- A randomized trial comparing a 30-minute outdoor forest walk to an indoor control found the outdoor group had greater improvements in attention and mood, larger reductions in salivary cortisol, and greater increases in parasympathetic activity (HRV).[7]
Enjoyment and adherence
Outdoor exercise is consistently rated as more enjoyable, which may support long-term adherence, though direct evidence on adherence itself remains limited.[5][3]
Practical takeaway
For patients whose goal is cardiovascular conditioning, treadmill and outdoor walking are largely comparable, with treadmill walking eliciting slightly higher oxygen consumption at matched speed due to biomechanical adaptation.[1] However, for patients also targeting mood, stress reduction, or exercise adherence, outdoor walking appears to offer added psychological benefit, though the certainty of evidence is limited by small trial sizes and heterogeneity across studies.[2][4][5][6][7]
